Life Care Planning

A Life Care Plan provides you peace of mind, knowing that your loved one who is in need of Long Term Care, is receiving appropriate care: protecting and enhancing well-being until the end of life.

Your Life Care Plan includes the services of your Elder Law attorney, Geriatric Care Manager, and other members of our staff who work together to coordinate care, maximize quality of life and protect assets for the loved one and the future generations.

A Life Care Plan is appropriate where your loved one has faced a life event, such as a health crisis, and is in need of Long Term Care services. It may be that your loved one is able to remain in the home, but life events have made it clear that care should be provided. Whether your loved one receives care at home or at a Long Term Care facility, a Life Care Plan will ensure that legal, social, financial, and most importantly, care services are appropriate, thus enhancing your loved one’s life-until the end of life.
